East and Southeast Asia are the world's largest markets for synthetic drugs. With rising demand for methamphetamines, health is not the only thing to suffer. The drug trade is also slowing development, say experts.
The use of synthetic drugs, particularly methamphetamines, is escalating rapidly in East and South East Asia. Powerfully addictive stimulants, methamphetamines can deliver a euphoric high but come with devastating side effects including brain damage, psychosis and severe decay as well as loss of teeth known as "meth mouth" - a tell-tale sign which could become increasingly recognizable across the region.
In East and South East Asia, the drug is mostly consumed in the form of pills, or by smoking white crystals known as "ice" - notorious around the world as crystal meth. Following crackdowns over the last decade on plant-based drugs in the region, most notably opium, the market has opened up for the production of synthetic drugs, which is more difficult to detect as it does not rely on cultivating large areas of land.
Myanmar, which remains the world's second largest opium producer after Afghanistan, is the principal producer of synthetic drugs in Southeast Asia. On August 18, the Myanmar government signed a landmark agreement with the United Nations Office on Drugs and Crime (UNODC) to implement a program tackling the country's major drugs and crime issues. The country is also currently hosting a regional conference aimed at indentifying solutions to resolve the problem.
Jeremy Douglas, UNODC regional representative for Southeast Asia and the Pacific, told DW that a transnational approach is essential when tackling synthetic drugs. One of the reasons the market has expanded so quickly is because economic integration in the region has created opportunities for drug-related activities across borders.

Keeping up with demand
While the level of plant-based drug use across East and Southeast Asia has remained relatively stable, "synthetic drugs have really exploded," Douglas said. "They've been very attractive to young users where opium or heroin is associated with the older generation," he added. And the use of the drugs has not only grown geographically - their social appeal has also expanded. "More recently they've evolved again and gone upmarket, attracting different demographic groups," Douglas told DW.
According to a report by the Australian National Council on Drugs (ANOD), "most countries of Asia are either producing countries or on major transit or trafficking routes from producers to consumer nations." The growing demand for methamphetamines in East and South East Asia is being met primarily through large-scale production in China and Myanmar.
Between 2009 and 2011, the total number of dismantled laboratories in the region that were producing amphetamine-type stimulants rose by almost 90 percent, according to a UN report this year. Douglas believes the key to stalling the trade is to target the precursor chemicals used in the preparation of these drugs. "Most of the countries in this region don't produce the chemicals that are necessary to make the drugs," he told DW. These chemicals are produced by pharmaceutical companies in countries such as China and India, and are then sold by traffickers to drug producers in East and Southeast Asian nations.

The UNODC estimates that the trade of synthetic drugs alone generates almost 17 billion USD a year in East and Southeast Asia. "This money is in the hands of criminal groups which are able to influence public officials; they are able to launder the money into legitimate businesses so it has a distorting effect on the economy," explained Douglas, adding that the huge economic power of the drugs trade can seem an attractive prospect for those involved.
After contacting a person familiar with the drug trade in China, DW found out that while "ice," or crystal meth, was previously the most popular methamphetamine on the market, a new variety nicknamed "pork," which is "stronger," has now taken over. "Nobody takes "No. 4" [high purity Heroin] anymore," the contact told DW on condition of anonymity, adding that the price of "pork" has also increased. The drugs are produced in China's Sichuan province, close to the border with Myanmar, he said.
While some East and Southeast Asian countries do offer treatment for drug addiction - Thailand, in particular, has extensive programs. However, the treatment available is often not in line with international recommendations. The focus on confinement and withdrawal often results in high relapse rates, where community-based programs show longer lasting results.
While the punishment for possession varies, almost every country in the region enforces the death penalty for the trafficking of methamphetamines, depending on the amount of drugs involved.

Undermining development
While poverty is believed to be the underlying cause of the surge in the market, the economy suffers even further from the ensuing financial burden. According to experts, the drug trade is a major drag on development in the region, drawing resources away from where they are most needed.
"A disproportionate amount of public expenditure is spent on the drug problem," said Douglas. Law enforcement bodies are under pressure, court systems are clogged with cases, prisons are overrun and in some cases the health systems are struggling to cope with the number of drug related patients, he explained.
Criminal activity on account of the drug trade is not only undermining development efforts, but also increasing human insecurity and threatening peace processes in the region, according to UNODC. The impact of the region's methamphetamine habit reaches further than the growing number of users and producers. The powerful drug trade is chipping away at the region's stability, even for those to whom "pork" and "ice" still just mean meat and water.

71% Use Synthetic Cannabinoids To Avoid A Positive Drug Test

More than one-third (38%) of adult patients in a Midwestern substance use disorder residential treatment program reported ever using synthetic cannabinoids.
Nearly all of these patients reported multiple reasons for use, with the most common being “curiosity” (91%) and “to feel good or get high” (89%). A substantial number also reported that they used synthetic cannabinoids “to relax or relieve tension” (71%) or “to get high without having a positive drug test” (71%; see figure below). Prior studies have also found that synthetic cannabinoids are used to avoid positive drug tests (see CESAR FAX, Volume 22, Issue 27). According to the authors, “consuming synthetic cannabinoids can complicate the treatment process, especially when urine tests do not identify all synthetic cannabinoids” (p. 2). The authors note that a limitation of their study is that it “lacked toxicology screening to verify recent substance uses or whether the substances patients reported were synthetic cannabinoids were actually synthetic cannabinoids and not another substance” (p. 3).

NOTE: Reasons were assessed with a checklist of 13 items developed by combining motives from Monitoring the Future’s marijuana motives questions and prior research on synthetic cannabinoid use.

OxyContin abusers switching to heroin

A change in the formula of the frequently abused prescription painkiller OxyContin has many abusers switching to a drug that is potentially more dangerous, according to researchers at Washington University School of Medicine in St. Louis. The formula change makes inhaling or injecting the opioid drug more difficult, so many users are switching to heroin, the scientists report in the July 12 issue of the New England Journal of Medicine.

For nearly three years, the investigators have been collecting information from patients entering treatment for drug abuse. More than 2,500 patients from 150 treatment centers in 39 states have answered survey questions about their drug use with a particular focus on the reformulation of OxyContin.

The widely prescribed pain-killing drug originally was thought to be part of the solution to the abuse of opioid drugs because OxyContin was designed to be released into the system slowly, thus not contributing to an immediate “high.” But drug abusers could evade the slow-release mechanism by crushing the pills and inhaling the powder, or by dissolving the pills in water and injecting the solution, getting an immediate rush as large amounts of oxycodone entered the system all at once.

In addition, because OxyContin was designed to be a slow-release form of the generic oxycodone, the pills contained large amounts of the drug, making it even more attractive to abusers. Standard oxycodone tablets contained smaller amounts of the drug and did not produce as big a rush when inhaled or injected.

Then in 2010, a new formulation of the drug was introduced. The new pills were much more difficult to crush and dissolved more slowly. The idea, according to principal investigator Theodore J. Cicero, PhD, was to make the drug less attractive to illicit users who wanted to experience an immediate high.

“Our data show that OxyContin use by inhalation or intravenous administration has dropped significantly since that abuse-deterrent formulation came onto the market,” says Cicero, a professor of neuropharmacology in psychiatry. “In that sense, the new formulation was very successful.”

The researchers still are analyzing data, but Cicero says they wanted to make their findings public as quickly as possible. The new report appears as a letter to the editor in the journal. Although he found that many users stopped using OxyContin, they didn’t stop using drugs.

“The most unexpected, and probably detrimental, effect of the abuse-deterrent formulation was that it contributed to a huge surge in the use of heroin, which is like OxyContin in that it also is inhaled or injected,” he says. “We’re now seeing reports from across the country of large quantities of heroin appearing in suburbs and rural areas. Unable to use OxyContin easily, which was a very popular drug in suburban and rural areas, drug abusers who prefer snorting or IV drug administration now have shifted either to more potent opioids, if they can find them, or to heroin.”

Since the researchers started gathering data from patients admitted to drug treatment centers, the number of users who selected OxyContin as their primary drug of abuse has decreased from 35.6 percent of respondents before the release of the abuse-deterrent formulation to 12.8 percent now.

When users answered a question about which opioid they used to get high “in the past 30 days at least once,” OxyContin fell from 47.4 percent of respondents to 30 percent. During the same time period, reported use of heroin nearly doubled.

In addition to answering a confidential questionnaire when admitted to a drug treatment program, more than 125 of the study subjects also agreed to longer phone interviews during which they discussed their drug use and the impact of the new OxyContin formulation on their individual choices.

“When we asked if they had stopped using OxyContin, the normal response was ‘yes,'” Cicero says. “And then when we asked about what drug they were using now, most said something like: ‘Because of the decreased availability of OxyContin, I switched to heroin.'”

These findings may explain why so many law enforcement officials around the country are reporting increases in heroin use, Cicero says. He compares attempts to limit illicit drug use to a levee holding back floodwaters. Where the new formulation of OxyContin may have made it harder for abusers to use that particular drug, the “water” of illicit drug use simply has sought out other weak spots in the “levee” of drug policy.

“This trend toward increases in heroin use is important enough that we want to get the word out to physicians, regulatory officials and the public, so they can be aware of what’s happening,” he says. “Heroin is a very dangerous drug, and dealers always ‘cut’ the drug with something, with the result that some users will overdose. As users switch to heroin, overdoses may become more common.”

Funding for this research comes from the Denver Health and Hospital Authority, which provided an unrestricted research grant to fund the Survey of Key Informants’ Patients (SKIP) Program, a component of the RADARS (Researched Abuse, Diversion and Addition-Related Surveillance) System.
